Please complete the COVID-19 disclaimer and medical condition info. You may send it to email@example.com prior to the first workout or you may turn it in to your coach on the day of the first workout. Individuals with certain conditions may not be allowed to participate in activities. You will not be allowed to participate in practice without this form.
WORKOUTS ARE NOT MANDATORY. THERE WILL BE NO REPERCUSSIONS FOR NOT PARTICIPATING.
ATHLETES THAT DO NOT PASS THE SYMPTOM ASSESSMENT MUST RETURN HOME IMMEDIATELY.
Athletes must bring their own water bottles. Bottles left after a workout will be disposed of.
Physicals from 2019-2020 are good until July 31st. Greeneville City Schools must have a record of your physical from last year or a new one for the 2020-2021 season to participate in summer workouts. Physicals for the 2020-2021 school year are acceptable only if they occurred after April 15, 2020. GHS will not be hosting mass physicals this year. Each athlete will have to have an individual physical scheduled.
Athletes should arrive dressed for workouts. No locker rooms will be available.
Drop Off/Pick Up
Temperature and symptom assessment will begin 15 minutes prior to the beginning of the group workouts. Athletes should not be on campus prior to that. Athletes should leave campus immediately after workouts. Athletes must park in their assigned parking area.
Symptom assessments will be completed on each athlete, each day.
No spectators may attend/watch workouts.
Schedules could change at any time. Players will be notified of schedule changes.
