Back-to-School Health Information Reminder

Make sure you selected Yes during registration so your child can be seen by the GHS Nurse for needs other than emergencies. If you would like to change that or would like more information, please contact Nurse Cassidy. 

Keep your contact and email information up to date with the school. When something changes, please let us know so we can prepare.

We do offer an optional over the counter medication program that you must sign up for and pay a $5.00 fee. It is completely optional but could help keep your child in the classroom learning environment for minor inconveniences that may not be medically excused. 

As we prepare for the 2026–2027 school year, we ask all families to update their child's health information and submit any necessary medical forms.

If your child has asthma, allergies, seizures, diabetes, or any other medical condition that requires care at school, please provide updated health care plans, emergency action plans, medication authorization forms, and any required medications. Medical forms and information for the previous school year do not carry over and are not valid for the 2026–2027 school year. New forms must be submitted for each school year.

Please note that some health care plans and medication authorization forms require a physician's signature before they can be implemented at school.
