Below is the list of items that we will need BEFORE your student can get registered for classes. These documents can be uploaded in the online registration (preferred submission option).

1. Copy of the birth certificate

2. Parenting plan/custody docs (if parents listed on birth certificate are no longer married)

3. TN Immunization Certificate (health physical signature if coming from out-of-state)

4. 3 items for proof of residency (must have all three):

   1. Mortgage statement or lease

   2. Utility bill

   3. Valid photo ID w/address

5. Grades, attendance, and discipline report for the current and previous school year

6. Complete the New Student online registration at: https://www4.gcschools.net/page/online-registration

IF YOU ARE A TUITION APPLICATION- You must also provide the following three items AFTER the current school year has ended:

• a high school transcript (or final report card from the 8th grade showing successful completion of middle school)

• attendance report

• discipline report (even if it is blank)

• Affidavit of Residence Form

  • This form is only needed if you are living with a family member or friend. If you have a lease or mortgage, you do not need this form.

  • If you need this form, the homeowner will also need to provide the residency documents listed above (ex. Mortgage/lease, utility bill, ID) in addition to this form.
